The RN Care Coordinator will be responsible to facilitate, in collaboration with the healthcare team, the patient’s plan of care to ensure that all services are provided in a timely manner and promotes a safe and appropriate discharge plan.  RN Care Coordinator will help to manage progression of the patient’s treatment from admission to discharge. They monitor and coordinate treatment plans with social workers, physicians, physical therapists and other medical staff to provide crucial case review assessment to improve patient care outcomes, improve access to care, decrease healthcare cost, prevent hospital readmissions, and promote continuity of care.

Qualifications include a valid NYS RN license and a minimum of two (2) years nursing experience.  Case Management experience preferred.  Excellent written and verbal communication, critical thinking, facilitation/coordination, and problem solving skills required.  Must be able to work proficiently with care management software platforms, electronic medical record and similar systems for documentation, patient monitoring and reporting and knowledge of healthcare delivery systems.  Candidates will work a full time or part time schedule with rotating weekends (one per month) with flex days off during the week.
