Care Management Department
The Care Management Department has two different types of personnel: Nurse Case Managers and Social Workers.
Nurse Case Managers are Registered Nurses with many years of experience. The Nurses review charts to ensure the patients meet criteria for admission and what is being done for the patient meets the current Standards of Care. They also talk to the insurance companies to justify stay based on acute criteria.
Social Workers are people who have degrees in Sociology, or some type of Human Services subject area. The Social Workers are non-clinical people who work with patients with a variety of problems, mostly discharge planning needs: Services needed in the home, physical therapy, oxygen, IV meds, nursing support, equipment. They also work with patients and families to place patients in higher levels of care: Nursing homes or adult homes.
Department provides 24 hr/day, 7 days/wk coverage for social work problems and discharge needs, for example: Child and adult abuse or patient placement.

Is the organized work intended to advance the social conditions of a community, and especially of the disadvantaged, by providing psychological counseling, guidance, and assistance, especially in the form of social services.
