Health Information Management
The Health Information Management (HIM) Department is responsible for safeguarding your health information from loss, destruction, tampering and access by unauthorized users. All patients have the right to privacy of health information concerning their care and treatment. This right is protected by organizational policy, medical record ethics and State and Federal laws. Providing adequate safeguards to prevent unauthorized access to the medical record and to prevent unauthorized release of information contained in the medical record protects patient confidentiality.
Auburn Community Hospital (ACH) HIM Department launched Horizon Patient Folder (HPF) an Electronic Health Record (EHR) system on March 31, 2010 as the legal medical record. HPF stores all patient health information electronically allowing immediate and simultaneous access to the patient’s health information by all caregivers improving patient care for the Auburn and Cayuga county community.

Monday through Friday from 8am – 4pm
Requests for health information or copies of your medical record need to be made in advance by calling the Medical Records Release of Information Desk at (315) 255-7322. Requests for Correction and/or Amendment of your Health Information must be done in writing in the Health Information Department.
Patients are required to sign a Release Of Information (ROI) Authorization form to receive their Health Information at time the information is received.
Patients wishing to view their health record on-line should call the Health Information Manager at (315) 255-7254 to set-up an appointment. For copies of your health information, other than information being sent to another care provider for continuing care, there may be a minimal charge for copy services.
